CASE STUDY- Patient Patricia.
An 80-year-old female patient named Patricia (name has been changed to maintain confidentiality) under the Nursing and Midwifery Council, NMC (2018).
Present on the surgical ward with bowel obstruction problem which needed surgery. Patricia has diarrhoea caused by Clostridium Difficile (C.diff). C.diff is a bacteria that can infect the bowel and cause diarrhoea; it is commonly affected by people recently treated with antibiotics. The bacteria can spread to others, so patients are usually allocated a side room to avoid spreading to other patients. Patricia had a cannula in her right hand and a catheter in situ. She also had diabetes with a Capillary Blood Glucose (CBG) being too low at 3.7. She was mildly confused, screaming, afraid that people would hurt her; she needed her family. She slept half the night; by morning time, she was starting to get aggressive, trying to get out of bed and fighting the two healthcare professionals providing care for her. Eventually she had a fall.
The doctors were waiting for her to recover from the C.diff before they operated on her bowel. She stayed on the word for one week before she was discharged home.
The treatment of antibiotics for C.diff worked; she then had successful surgery. Her CBG was within range, the catheter and Cannula were no longer required, so it was removed. Finally, Patricia was discharged home with a care package of carers visiting twice a day.

IDEAS:
» Read pages 13-21, of the NMC Standard (2010)
» Evaluate patient care, break down about 7 competences and refer to them in depth on how they relate to your service user. (1 -2 competences for each domain)
» For example: Write about a domain, bring in one competence, line it with the NMC code without being judgemental, then do a critical analysis.
» Show why things are the way they are. What were the barriers? What can be done better?
» Critically evaluate what went on in terms of clinical governance e.g using NICE guidelines, policies, bench marking.
» How well did it help you to deliver the care you did? How successful was the outcome? Did the care fall short? Or fully embodied.
» The care management- talk about team working, using leadership, for example the process involved in discharging Patricia.
» Did you have to escalate anything? At what point? Why/ reflect. How did it impact the patient? Add evidence literature. Did you use specialist nurses?
» How is this case study different from other case studies? Look at this form a leadership view, management of care.
» Try and understand the process underpinning care. (No need to do a complete assessment of patient, or entire story).
» Select the competences to evaluate the leadership and management, analyse them, then relate them to the four domains.
» Use the competences to evaluate the leadership and management of care, understand why?
» Look at things which did not go well, analyse them rather than describe. How were they managed?
» Look at patient care - then cross reference it to 4 domains, policies.
» Give a balanced argument/approach, do not be too judgemental
» The essay is a critical appraisal, critical evaluation and critical analysis.
