Sales representatives of a large manufacturer of mechanical components are required to submit order forms to the production department for sales they have made. Often, the order forms are incomplete (e.g., catalog numbers of the components are missing, buyer's address missing). When order forms are incomplete, a clerk in the production department fills in some of the more important missing information and the order forms are processed as usual. Every two or three months, the production department manager gets upset and complains to the sales department. Each time, the sales department sends a memo to its field managers urging that they stress the importance of the complete order forms to the sales representatives. The order forms from the sales department usually are complete for several weeks following a complaint and a memo.
