Question 

You are caring for an elderly patient in the ortho unit whose family did not want him to take pain meds because they made him drowsy and he could not complete PT/OT goals and attain his discharge criteria. The family's stated goal was for the patient to go home as soon as possible because it was inconvenient to travel the distance to the hospital. The patient agreed with the family but then would ask for pain medication. Is this an example of moral uncertainty, moral distress, or an ethical dilemma? Do you think it is a different ethical issue if viewed from the perspective of the family, nurse, or patient? What would you do in this situation? What would you tell the family about attaining discharge criteria in this situation?
