Care Coordination is key in Nursing to provide safe quality care. It is important to communicate effectively between the interdisciplinary team to ensure the patient gets the necessary care. Effectiveness will reduce ER visits and readmissions. Processes may be put in place to assure positive patient outcomes, as we want to ensure the patient has a positive experience while hospitalized. These are times when a patient is going through their most vulnerable times and is trusting we will provide quality care. Engaging the family in the care, for example, explaining the treatment and educating the family for a safe transition home is vital. This will assist the patient to transition to the community and continue to care for himself with the assistance of the family. Assessing barriers, such as education, language, and financial barriers, will assist us in providing the necessary resources needed. As the article mentioned "care coordination interventions that bring together all health professionals involved in the patient's care can improve LOS and discharge time of day" (Park et al., 2023). Involving the health team will safely provide the care as everyone will have access to all communication about the patient. For example, the team will know about any current changes, allergies, or new treatments. Improving the length of stay can also reduce costs so that revenue can be used in the appropriate places or things for patient care. This article has information to benefit us as we want to provide quality care, have effective communication, and reduce unnecessary cost. I chose this topic because I think it is a vital topic in healthcare to provide safe quality care. I also believe we can always have a refresher on how care coordination can impact either positively or negatively. Care coordination involves the interdisciplinary team, requiring a team to work together. This will benefit the patient as we implement the appropriate interventions as a team. Nursing staff and the interdisciplinary team can benefit from the research. On a daily basis as we care for our patients, care coordination is key. From the beginning when we assess the patient, obtaining critical information that can affect the patient's care to educating the patient and family upon discharge. The entire team caring for the patient can benefit from considering the importance of care coordination. As nurses we want to give the patient the resources necessary to better their health and be compliant with the treatment. While under our care, we want to continue to monitor the patient's treatment and make sure it is therapeutic. Also teaching the patient how to continue to monitor the treatment at home and teach the importance of compliance. I believe the patient's needs need to be heard and we need to be aware of any cultural or spiritual influences. For example, assessing language barriers is important to me. Making sure the patient understands what we are asking and teaching is upmost important. If the patient does not understand our teaching, we can be putting up the patient for failure once out in the community.
