You arrive to your scheduled shift and begin your initial patient assessment. During your initial assessment you note a discrepancy in the breaths per minute the patient is receiving via the ventilator (16 bpm ) and the physicians order (12 bpm ) , as written on the most recent 485/ Plan of Care. The patient is in stable condition with no noted distress and all vital signs are normal. What should you do first? Immediately turn the vent rate to 12. Review the patient's medical record to identify and supplemental physician's orders reflecting this change. Call the physician to verify the ordered vent settings.
