Context: 12 y/o WF articulate female, presents with her Dad. She c/o R ear ache with decreased hearing. States her ear is itchy with talking and she is super tired. Denies fever, C/O Sl. runny nose but states clear d/c. No sore throat or rash identified. Yesterday she was nauseated but did not throw up. She c/o a headache that is relieved with ibuprofen. Otherwise she has not treated her symptoms but did stay home with Dad today and from school. She cont. to drink and eat, sleeps more though. Everyone at school has had strep throat. Denies visual change, chest pain, shortness of breath, abdominal pain, urinary symptoms, muscle or joint pain, rashes, numbness, tingling or weakness.

PE: R TM erythematous, bulging with middle ear effusion, Cl d/c at nares with atopic turbinates bilat. Rapid strep test neg., neuro exam 2-12 intact.
