Homework: Population & Community Health Nursing

For the PIPR paper you are asked to identify a relevant community health need that your project will address, to substantiate this need and to discuss the evidence base for potential actions to address this need. You are asked to identify theory-based approaches to address the need. Finally, you will be asked to describe the project plan. Please follow the ‘Components of Paper' outline below. Please note that if your project involves participation in a program or activity already planned by the clinical site you will still need to apply analysis to best identify, in the PIPR paper, the community need that you will be addressing and potential approaches to address the need.

Write the paper in APA style, include a properly formatted title page and reference page. Use the ‘Components of the Paper' below as organizing headings. The paper should be 6 to 9 pages in length not including title page or references. Support your content with at least six scholarly references. For this paper, epidemiological sources are considered scholarly sources however these should not make up more than half of your references. Use the scholarly sources to support why the actions that you propose in your plan are indicated.

Body of the Paper

I. Title of the Paper - Label the introductory paragraph with the title of the paper. This level 1 heading is centered, in title case with a bold font.

Problem Identification and Planned Response for [specify the concern and the population served]

II. Community Setting and Role (Level 1 heading)

1. Describe the community health setting (the practicum site and populations served) my practicum site is People with developmental disability. They are adults that live in a group home.

2. Identify the preceptor's title and role (My preceptor's title is Habilitation Specialist. His roles are Safeguards and goals for developmentally disabled individuals).

III. Problem Identification and Population(s) Affected (Level 1 heading)

1. Briefly state the community/population health problem and describe how this issue was initially identified as important for the community served by your practicum site.

2. Is there a population group(s) particularly affected by this issue? Are there issues of health inequity involved? Support these points.

3. Substantiate the importance of the problem with epidemiological statistics. In your analysis note the level of statistics cited (National, State, County, and/or City) and how this applies to the community served at your practicum.

4. Expand on your analysis of the nature of the problem with an integration of perspectives from relevant scholarly literature. The identified community/population health problem should be described and factors leading to development and/or exacerbation of the identified problem should be discussed.

5. Identify at least two local resources available (beyond your project) to the community for this issue.

IV. Potential Approaches to Address the Identified Problem (Level 1 heading)

Identify one to two potential actions at each of the following levels. These listed potential actions may be informed by scholarly literature and/or primarily based on your own idea. (see Figure 9.2 in our textbook)

V. Chosen Action/Planned Response in Relationship to Theory (Level 1 heading)

1. What is the chosen action you will take on for your practicum project?

2. How does your chosen action relate to health promotion and disease prevention? Is this action at a primary, secondary, or tertiary level of prevention (please explain).

3. What determinants of health are addressed by this action?

4. Does this action most directly involve change at a structural level or at the level of individual's behavior? How so? How does your planned action response relate to theory in one of the following areas: structural or program change; behavioral change theory; and/or to health teaching models?

VI. Chosen Action/Planned Response and Evidence Base for Response (Level 1 heading)

1. Discuss the evidence that supports the potential efficacy of this planned action. Are you able to locate an appraisal of the strength of this evidence?

VII. Planned Response/ Project Plan (level 1 heading)

Objective. What do you aim to be the immediate impact of the planned response/action project (achievable through the project and will contribute to addressing identified problem)?

1. List the series of activities that will be necessary to complete the larger planned action/project and achieve this objective.

2. Identify persons you will work with at each step/activity to accomplish the project. Who will hold responsibility for implementation and completion of each step/activity?

3. Note the planned timeframe for each step/activity in the project?

VIII. Plan for Evaluation (level 1 heading)

1. What is an ideal plan for outcome evaluation (without time or resource limitations).

2. What will be your plan for outcome evaluation (given time and resource limitations).

Note if planned outcome assessment addresses process outcomes (the program implementation process) or change outcomes (in knowledge, behavior, structural change, or health outcomes).

IX. Conclusion (Level 1 heading)

Provide a summary of key points (1-2 paragraphs)
