Question: Briefly describe the provider's interviewing skills. Consider referring to Carlat's text to describe his technique. What important subjective information is missing from the interview? (1 paragraph)

Write a mental status examination for Sarah. List three differential diagnoses for Sarah with a brief rationale for each diagnosis.

After reviewing your differentials, consider all factors that may be contributing to the patient's symptoms. Write a biopsychosocial assessment, also known as a case formulation, for this case. Include primary diagnosis around which to base the patient's treatment plan. (1 Paragraph)

Write your treatment plan as you would for a real patient. Include any medications, non-pharmacologic options, education, referrals, follow up, etc. Imagine your professor is a fellow PMHNP providing coverage for you and will need to review this note in a practice setting. You will want anyone else reading your notes to feel as though they have a clear picture of the patient just from reading your note.
