Question 1. List and describe 3 OSH issues that the incident highlights.

Question 2. For each of the 3 OSH issues you described above, list the designated persons or units that you could raise each of your OSH issues. Explain why you would raise any given issue with the particular person or unit you have listed. (e.g. maintenance department - to adjust slippery surface of linoleum floor by placing non-slip decals on it) Please do not use the same example as part of your answer. Question 3. "We'll be the best people to think out the solutions to the OSH things you mentioned. What solutions can you think of and how can we implement (put into practice) your good ideas?
For each of the 3 OSH issues you described above, describe how you would help develop a suggested solution and how you would help implement your solution. (e.g. select appropriate non-slip decals for floor involving Bob Crane and family for choices. Once type of decal selected, occupy Bob with activities outside of room whilst maintenance applies decals. Ensure Bob is acclimated to new flooring by showing it to him, letting him walk on it, asking him if he likes it etc.) Please do not use the same example as part of your answer. Question 4. Briefly describe why is it important to have ongoing training to maintain current knowledge of safe work practices concerning workplace systems (e.g. nurse call system, client/resident data base system etc.), equipment (e.g. full weight bearing hoist, slide sheet etc.) and processes (e.g. fire emergency procedure, hazard reporting process etc.) to do with your work role.
Question 5. List and describe at 3 types of training (either provided by your organisation or external to the organisation) that you have done to maintain current knowledge of safe work practices (in regards to workplace systems, equipment and processes to do with your work role).
Question 6. Briefly describe a workplace debriefing you may have participated in after an incident at your workplace (e.g. you may have sustained a workplace injury or encountered an aggressive client.)
Focus your description on how the issue was resolved and what kind of support you received from your organisation. If you have not participated in a workplace debriefing, based on your learning and experiences, describe the process you would expect to happen.
Question 7. Describe coping mechanisms you use to manage your level of stress and fatigue that you may experience due to the work environment (e.g. keeping a diary, meditation, extra sleep etc.)?
Question 8. If you become stressed and/or fatigued (whether or not related to the work environment), to whom and how would you report this status according to organisational policies and procedures?
