A brief reflection and intervention as the nurse for your patient who has one of the above

Step 1: Choose a patient who is living wither either Type 2 Diabetes or Hypertension.

Step 2: Choose a meal plan (DASH or Mediterranean Diet)

Step 3: Choose a patient profile: (a) veteran experiencing homelessness (b) pregnant mother living in a food desert (c) newly released parolee who was incarcerated (d) newly arrived refugee who lacks English fluency and has less than a primary grade's education

Step 4: reflection and intervention as the nurse for your patient.

Questions to consider in your reflection:

-Where will they shop (or find) for a day's worth of groceries?

-How and where will they prepare their meals?

-Where are they getting their income?

-What other questions or areas would you need to address or assess?

-What are other considerations you will need to take under consideration as it relates to their diagnosis?
