How could you best manage stable, chronic IHD for a 65-year-old African American male who first noticed symptoms six months ago. His shortness of breath seems to be getting worse with exertion and is not relieved with rest. The chest pain is intermittent and worse with exertion. OTC pain relievers are not sufficient to alleviate his chest pain. He states that his chest pain is a 6/10. He denies palpitations and syncope. The review of the remainder of his systems is negative. Past Medical/Social/Family History: The patient's medical history includes hypertension, diabetes mellitus, and obesity. No previous surgical history. The patient also states that he has smoked 2 packs per day for the last 20 years and drinks on average 2 beers per night. Family history includes hypertension and diabetes mellitus on both his mother's and father's sides of the family. Physical Examination: Findings are as follows: temperature of 98.9°F, blood pressure of 170/86 mm Hg, heart rate of 85 beats/min, respirations of 14 breaths/min. Body mass index (BMI) is 42 kg/m2

The patient appears fatigued and short of breath. His heart rate is regular, lungs are clear, and abdomen is soft. Pt has 2+ pitting.
