Assessment findings are: Lungs are clear to auscultation bilateral, heart rate is regular; breasts are soft and non-tender to palpation; last bowel movement was last night at 2000; voiding without discomfort. The fundus is 1 cm below the umbilicus, midline, and firm; lochia is red and scant; repaired mediolateral episiotomy is warm but non-tender, the hematoma is present but stable, with minimal clear discharge, and the distal edge is approximated. Hemorrhoid is present, lower extremities are symmetrical in size, denies calf pain, pedal pulses present and bilateral. The client reports mild discomfort of 2/10 in the perineal area. The client reports a loose bowel movement that started early this morning.
