Anna is a 50-year-old female with a history of hypertension for the past 10 years. Her doctor prescribed blood pressure medication as well as a Low Sodium diet. Anna has not consistently adhered to her medication or Low Sodium diet. She is 5'4" tall and weighs 150 pounds. Anna gained 5 pounds over the weekend despite a poor appetite. She became short of breath and noticed her feet were too swollen for her shoes to fit. She went to see her doctor and was diagnosed with Stage 5 Chronic Kidney Disease (CKD). She has oliguria and anemia. The doctor admitted Anna to the hospital for immediate hemodialysis due to fluid overload and a blood potassium level of 6.7. Additional lab values include: BUN 110 Creatinine 4.2 Albumin 2.1 Ferritin 30 Calcium 8.2 Phosphorus 7.0 Hematocrit 28.5 GFR 18 What is the probable cause of Anna's kidney failure?
