Sw explained to Sw takeya that the patient has been discharge from Caris Healthcare Hospice services due to pt signin g a revocation. pt stated she wanted to live for her grandchildren and seek medical treatment. SW explained that that Skilled nursing has declared pt not hospice apporiate and she would benefit from homehealth services.
