The chart entry for a client in the psychiatric emergency department reads: 2/10 1200 The client was cooperative and cheerful during the first part of the assessment. The client then responded strongly and arrogantly to questions, and became excessively talkative about "needing a new car to be an effective salesman." The family reported that the client was loud, pacing, gesturing frantically, and verbally threatening the client's parent for refusing to buy the client a car. The client admitted to "not taking my medications for the last three days" Client is admitted to the psychiatric hospital unit. Vital signs: T 98.6, P 100 R 22, BP 128/78. Based on this chart entry, what additional behaviors might the nurse observe after admission to the unit?

The client tries to speak to each staff member and other clients on the unit. When the nurse sets a limit, the client laughs and interrupts the nurse. Nonacceptance and negativity are apparent during the client's interactions. During unit orientation, the client becomes easily distracted by others.
