M.S. is a 38-year-old nurse who works with you in the surgical ICU. He went home this morning after working a 12 hour night shift and awoke early at 1300 with what he told his wife was "the worst headache I've ever had." He attributed the headache to being tired, took 2 ibuprofen, and went back to bed. When he got back up at 1700, he said he still had a severe headache. M.S. took a shower and sat down to eat dinner. As his wife was placing food on the table, he grasped his head and fell onto the table, unconscious. When the paramedics arrived, he was unresponsive. They intubated him and started an IV with normal saline. On arrival at the emergency department, his Glasgow Coma Scale (GCS) score is 3. His wife, who is a nurse on the medical floor, says M.S. has a history of hypertension. Otherwise, she says that he was in good health. He drinks alcohol socially on weekends. She says that he had been having intermittent headaches for the past few weeks, which they attributed to stress due to short staffing and long work hours.
