At Edison High School, case managers assist the school with identifying their resources and helping them link with community resources. There is a student health and human service action team, where the school and the community come together. The case manager makes sure that each student receives the services needed. For example, if a teen is pregnant, that teen receives post-pregnancy planning and health and mental health care. Ultimately, the parents and the teens themselves assume the case management responsibility.

The following seven examples present case management services. Identify the method described and a minimum of two roles that will be necessary.
