Sandra is normally a very alert and oriented older woman who participates in activities and often goes out of the assisted-living facility on shopping trips with her daughter. Today you notice she is acting differently than normal. She does not recognize you when you enter her room, even though you care for her every day. She starts to talk about the cat that has been roaming around her room all night, keeping her awake, although there is no cat there. When you ask Sandra if her daughter will be picking her up to go shopping, she tells you that she has no children. You take a set of vital signs, and all are normal except her temperature, which is 100.1°F.
