Febrile Seizures

Isaac Hernandez is a 15-month-old boy who was brought to the emergency department with a seizure associated with fever. He has been in good health until today when he developed a fever of 104.8°F taken rectally. His mother gave him 1 teaspoon of acetaminophen 100 mg/5 mL. About 15 minutes after she gave the acetaminophen, she noticed shaking of his arms and legs, his eyes had a blank stare, and he would not respond when she called his name. His mother called 911 and an ambulance arrived in 5 minutes. When the ambulance arrived, the seizure movements had stopped, but Isaac was not fully awake. He was transported to the emergency department by ambulance and on arrival there was awake and somewhat irritable. His temperature on arrival was 100.2°F rectally, his heart rate was 100, respiratory rate was 26, blood pressure was 76/50 mm Hg, and oxygen saturation was 100%. He weighs 22 pounds, and his height is 31 inches. His physical examination was normal. He calmed considerably and was easily soothed by his parents. Isaac became playful and interactive with his family and the nursing staff in the emergency department.
