As the nurse practitioner for an inpatient psychiatric unit, you are present when the staff must forcibly restrain a psychotic client who has assaulted a peer. Further legal action is unnecessary if the client was admitted volulntarily. The client must be observed by staff every 30 minutes. The client in restraints and seclusion must be continuously observed and checked according to state and federal protocols for circulation, respiration, nutrition, hydration, and elimination.
