PT: Abraham Lincoln Provider: Barack Obama, MD Preop DX: Upper GI bleed Postop DX: Antrum gastritis with superficial ulcerations Nonbleeding duodenal bulb ulcer PX: EGD INDICATION: A 54 yo white alcoholic male with pneumonia and respiratory failure. Abraham presented with blood from his NG tube. His hgb was 8 but has fallen to 6.5. Today's plan is an EGD to rule out esophageal varices. PX: A flexible Pentax video endoscope was passed into his oropharynx and the gastroesophageal junction was seen. We performed inspection of the esophagus which showed no ulceration or erythema. We then approached and entered the stomach and in the antrum, there was redness, and some non-bleeding ulcerations. We then entered the pylorus and could found a deep duodenal bulb ulcer. After this in the post-bulbar ulcer was a large superficial non-bleeding post bulbar ulceration. The patient tolerated the procedure well. Dx: Gastritis Duodenal bulb ulcer Stomach ulceration Plan: Continue IV and repeat hemoglobins. what are the ICD 10 codes?
