Case: You are a FNP in a community-based clinic and you have had a chance to analyze your patient population and concluded that you have been building a large group of postpartum patients. Interestingly, you have noted that 15% of your patients have been experiencing postpartum depression. One of your patients arrives for an appointment 6-week post-partum and shares with you that she is concerned that she may not be "loving" her baby like other mothers. She is 28 years old without any significant medical history, she has two children, 4 and 5 years old. She has given up breast feeding because she began feeling incredibly stressed caring for the baby and her other two children. She confides in you that she has not informed anyone else of how she has been feeling. It is important that you read the chapter to get a good understanding of Transition triggers, properties of transition, and the patterns of responses. One of your differential diagnoses is post-partum depression.

The care plan is based on the patient scenario provided. This week, as always you are being asked to provide your understanding of Meleis' theory and its application to nursing. Review Meleis' Transition Theory and develop a care plan utilizing a similar format (as noted in the exemplar on pages 366 -377).
