Case: You are a seasoned nurse in a skilled nursing facility that has over 30% new staff. Orientation training has been shortened due to the critical need for staff. There has been an increase in the number of falls on both units. Additionally, you've noted an increase in the severity of falls, including a patient fall resulting in a fracture and another patient fall resulting in a subdural hematoma. Both patients were hospitalized and required surgery. During rounds, you have noticed that fall mats previously used with fall risk patients are now in use with patients who are not fall risks. Additionally, the units lack enough bed alarms and wheelchair alarms. Do You Need Help?

A committee has been formed to address these issues. Included on the committee is a nurse manager, a pharmacist, a patient care technician, a staff educator, and an individual from the quality department. Your nursing manager has asked you to compile two performance gaps to be presented to the committee.
