Case - Usha, 28 years, is a married lady. For the last two months, Usha has been affected by continuous worry. She has also found herself compelled to repeatedly perform some actions and activities. Her behaviour has caused much anger to her husband and he forcibly took her to a doctor. The doctor referred her to a mental health professional.
During her intake interview, Usha refused to believe that her repetitive behaviour is a problem. Although she notices that these behaviors are relatively new, she does not believe that it should be of anyone's concern. When asked to elaborate on her repetitive behaviour, she said that she feels compelled to repeat her prayers three times in the afternoon, and that her husband feels annoyed as he has to sit through them. She fears that her mother would die if she does not do her prayers in this way, and claims that she is only doing this to ensure her mother's protection. When probed further, it was also revealed that she feels better if she checks the locks seven times, and washes the plate five times each time she eats. She is certain that all these measures are taken for the protection of her family. She also likes to ensure that lights are switched off properly by repeatedly checking the switch. Usha also worries very much about leaving the gas cooker switched on as this would cause a fire. She reported a similar incident happening when she was younger. Thankfully, her mother had seen it on time and switched it off.
Usha's mother is described to be a very anxious lady. She was very worried about her family as her husband lived in Italy, and she was responsible for the safety of her family. She was a methodical lady who insisted that Usha grows up the same way, and Usha was often punished for acts of carelessness. Usha reported still remembering these punishments. When discussing her family, Usha also mentioned that she has a sister diagnosed with anxiety and major depressive disorder.
Usha's mother was recently hospitalized after being diagnosed with dengue. Her older age made recovery a painful process, and she was often extremely irritable and angry. Usha reported being much more stressed since her mother fell ill as she became her mother's sole care provider. She said her worry becomes the worst when she has a disagreement with her mother, or when she feels that her husband does not care about her. She also feels worse when noticing that her mother's recovery is slow.
When she has some free time, Usha likes to read and become more knowledgeable about things. She enjoys singing, and spending time with her baby daughter. Prior to her mother's illness, she was also able to go out with her friends and exercise every evening. Despite her complaints about her husband within the first session, Usha describes their relationship as loving, and considerate of each other. She says that he is supportive of her in her decisions, and often willingly helps her with the care of her mother.

Case 1 You are a health psychologist working in an NGO that specializes in substance addition, particularly smoking cessation. Saman has come to obtain your support to stop smoking as his family doctor has recommended that he quits smoking. When you first assess Saman, who is 37 years old, he tells you that he has no current health problems. He smokes 25-30 cigarettes a day. Saman has a Body Mass Index of 17.9 and works as a hotelier and hence works long and irregular hours, including working late into the evening and on weekend, depending on the functions taking place at the hotel. He has unpredictable working hours and is constantly on pins (feeling nervous and agitated) when he is in charge of large events at the hotel. At times like this, he says that he frequently smokes instead of eating. He is aware that he does not live a healthy lifestyle but also reminds you that his grandfather, who was a heavy smoker, lived to be 93 and was healthy throughout his life.
