Task

1. Analyse the functional movement capacity of the case study client by observing their static and dynamic posture as follows:

a. Static posture (anterior, posterior and both lateral views)

b. Dynamic posture: Joint mobility
a) Shoulder Flexion / Extension
b) Shoulder Abduction / Adduction
c) Trunk Lateral Flexion
d) Trunk Forward Flexion

c. Dynamic posture: Functional movement patterns
e) Overhead Squat
f) Lunge
g) Seated Trunk Rotation
h) Push
i) Pull
j) Overhead Press
k) Overhead Pull

2. Give an example of a postural dysfunction you identified in the static posture observation that impacted on the client's functional movement during the dynamic posture observations. How did it limit the client's functional movement?

3. Identify the short/tight muscles that may be contributing to the movement dysfunction you identified in Question 2:

4. Provide examples of specific stretches or mobilization techniques you would consider including in the client's program to reduce the risk of injury and restore the function of muscles in Question 3:

5. Identify the long/weak muscles that may be contributing to the movement dysfunction you identified in Question 2:

6. Provide examples of specific activation resistance exercises you would consider including in the client's program to reduce the risk of injury and restore function of the muscles in Question 5 :

7. Provide examples of exercise precautions or specific exercises/activities that are contraindicated (not advised) for the client, based on your example in question 2.

8. Based on all your postural observations, would you refer this client to an allied health professional?
Yes No

If yes, complete the referral template form (included in this assessment) and request guidance from the appropriate allied health professional.

9. What would you say to the client to inform them of the legal and ethical limitations you have as a PT when observing and addressing functional movement capacity?

10. You receive the following information back in response to your referral to the allied health professional:

To the Personal Trainer,

Thank you for providing detailed information about the postural and functional limitations you have observed with Todd. I have since assessed Todd and have concluded that he is showing multiple signs of poor posture, which may progress into pain, degeneration and loss of function if not addressed promptly.

It is important for Todd to continue a regular exercise plan consisting of both strength exercises and mobilization exercises to improve function and flexibility. Todd would specifically benefit from muscle activation exercises that separately target each of the following muscle groups:

- rotator cuff (external rotators)
- lower trapezius
- pelvic stabilisers such as gluteus maximus

and also requires mobilization of the following areas, which I would recommend through a combination of static, dynamic and PNF stretching or SMR foam rolling::

- the anterior shoulder joint (chest)
- hip flexors
- lumbar spinal erectors

I will continue to monitor Todd over the next few weeks. Please keep me informed of Todd's progress with the exercise plan.

Regards,

Damien Farrell Osteopath

11. Based on your observations and the response from the allied health professional, design a 45-minute exercise program you can deliver for the client to promote functional movement and reduce the likelihood of injury.

Use the template on the following page to record your program. For each exercise/stretch, include a specific reason as to why you have prescribed the activity (eg a PNF chest stretch to stretch pec major and mobilise the shoulder)

12. What advice would you give to the client to educate them about how they can make modifications to their activities of daily living to assist promote good posture and function?

13. Have a friend, family member or class mate play the role of the client and conduct the training session for the client.

During your session:
a) Modify any incorrect technique
b) Correct any poor posture displayed by the client when performing the exercises
c) Explain to the client the importance of each exercise in relation to posture and function and the impact the exercise has on various muscles and joints.

14. Following the session, have the client complete the session evaluation form included in this assessment. Include the completed electronic forms for submission with this assessment or alternatively scan and upload the original hand written copy.

Based on the feedback from the client, evaluate your session plan by answering the following questions:

a. Which exercises did the client display poor posture/technique and how did you address it?

b. Were there any exercises that you would need to modify for future sessions? Explain:

c. Were the activation and mobilization exercises you prescribed effective in targeting the intended joints/muscles? Give examples and explain how you knew they were/weren't effective:

d. What exercises did the client find most difficult and why?
