Case:

Mr. Smith is an 80-year-old civil engineer widower with a history of type 2 DM with a HbA1C above 9 and secondary cardiovascular disease. He takes Glucophage 1000 mg BID and Enalapril 25 mg BID. At his last physical his blood pressure was 148/86, his BUN and Creatinine are within normal limits.

He lives alone in the home he once shared with his two children and his wife, who passed away five years ago. He will drive only to and from his doctor visits, and much of his time is spent indoors. Driving and caring for his three-bedroom home and yard is increasingly difficult because he now has physical signs of leg pain and is unsure of his ability to ambulate safely. He stopped taking daily walks and quit going to the gym two years ago after he fell in the parking lot. His meals are delivered by Meals on Wheels and he often orders food from the local pizza and Chinese restaurants. The highlight of his week is the call he receives from his children living in California and Alaska. His children suggested moving but he is not sure if he would be able to transfer his health insurance and state pension plan to another state. He has Social Security and Medicare benefits. He also has a small amount of savings ($250,000) set aside. At his last well visit, he mentioned to the nurse that he sometimes talks aloud to his wife, and some nights he is unable to sleep.
