A 72 year old African American female client came to the clinic and presents with a blood pressure of 172/94 mmHg, RR 18, PR 88 bpm, Height 5'2, weight 201 lbs, and afebrile. Medications: Multivitamin and Synthroid 75 mcg one tablet daily. Past medical history includes Type 1 Diabetes Mellitus and hypothyroidism. Past surgical history includes cholecystectomy 3 years ago. Social history: lives alone in her own home with her dog. Smokes a pack of cigarettes a day x 30 years. She reports she mostly stays at home and watch TV and Netflix. Her grandson comes every other day to walk her dog.
