Question

Nausea related to adverse effects of chemotherapy or radiation therapy as evidenced by aversion toward food, increase in salivation, increase in swallowing movements, gagging sensation, or sour taste Goal/Outcome Child will experience decreased nausea: will verbalize symptom relief and will be free from vomiting. Alleviating Nausea and Vomiting Administer antiemetics prior to chemotherapy and as needed thereafter to decrease frequency of nausea. Assess frequency of vomiting and level of hydration to provide baseline data and recognize alterations early. Offer frequent, smaller meals or snacks: smaller amounts are less likely to be vomited. Avoid spicy foods to avoid stomach upset. Allow bubbles to dissipate from carbonated beverages before they are ingested: carbonation may contribute to nausea. Remove cover from meal tray before entering child's room: this will allow the food odor to dissipate outside of the room; food odors may trigger nausea and vomiting. Nursing Analysis Alteration in nutritional status related to insufficient dietary intake as evidenced by food aversion, food intake less than recommended daily allowances, or weight, length/height, and/or body mass index (BMI) below average for age or individual child's usual measures Goal/Outcome Child will improve nutritional intake, resulting in steady increase in weight and length/height. Promoting Adequate Nutrition.
