Mom leaves in North Carolina and both parents has custody. Patient was recently discharged from inpatient hospitalization due to depression. Upon discharged from the psychiatric hospital, patient prefer to go leave with her mom in North Carolina. Step mom reports patient's mom is a kleptomania and did not follow the treatment plan. Therefore, patent became depressed, manic, weight gain, sad, dropping grades, hearing voices, and paranoid about people judging her or upset with her. Laboratory requisition made and discharged medications were refilled, and appointment scheduled for the next 2 weeks and consent was signed by her step mom. Patient's meds are ability, intuitive, Lexapro, melatonin, and hydroxyzine.
