You are the WOC nurse in an acute care setting. Mr. Jones, a 78 year old male, is admitted for pressure injury care. He has been cared for at home by his 75 year old wife. Mrs. Jones has poor eyesight and rheumatoid arthritis. Assessment: Mr. Jones is thin and contracted. He makes eye contact and moans when repositioned, but is unable to communicate. His skin turgor is diminished and he has an indwelling catheter in place. He has two pressure injuries:

1. Right trochanter: size is 8 x 8 cm, with firmly attached eschar. There is minimal surrounding erythema, the area does not appear to cause Mr. Jones pain. There is no drainage.

2. Left trochanter: 8 x 10 cm with eschar totally covering the wound bed, foul odor, and moderate drainage. There is erythema and induration extending 4 cm circumferentially from the ulcer.
