Question

A home health nurse is performing an admission visit with client after hospital discharge. Nurse enters the home and notes there is a significant amount of clutter. There are stacks and stacks of magazines in the living area along with what appears to be old mail and paperwork. Nurse also notes the client is caring for at least 20 cats that are currently able to be visualized. There is one chair in the living room that can be utilized but most of the other furniture is covered in paper and boxes. Home smells of urine and feces. When nurse asks about the state of the home and pets, client states they get "severely anxious when trying to throw their important possessions out."
