Question

Rephase. PC from PCg reporting pt is declining and only wants morphine. pcg reported she has been administering medications that they had left over from last hospice readmission before pt decided to revocate every hour and he has decided not to return to the hospital and again and promises he will commitee to the hospice plan of care. sw explained to pcg that the patient will have to have an assessment and evaluation to determine if pt is hospice apporiate and that no medication refills can be provided until the nurse is able to asess the patient apporiateness for hospice. SN notified and sW also shared concerns with SN about possible drug seeking.
