Question

The nurse prepares to administer medications via a gastrostomy tube (G-tube) and notes the measurement of the incremental marking is 0.5 cm difference from what is recorded on the medical record. What action should the nurse take? Do not aspirate or flush the tube and notify the healthcare provider immediately. Aspirate and note if tube feed is present as this verifies placement in stomach. Measure pH of aspirated contents and if more than 5.5, this verifies placement. Measure pH of aspirated contents and if less than 5.5, this verifies placement.
