Case Scenario
Care of Client with Ostomy
Client: Mrs. Apolo a 63-year-old female admitted 3 days ago post Ostomy Surgery for recurrent rectovaginal fistula. The ostomy is temporary to help the bowel rest, and clear infection to have the fistula repaired. She is an avid swimmer and does laps at the local pool 3 times per week, and aqua aerobics 3 days a week. She is married, her husband manages the local Home Depot and works shift work. She has 3 adult children who live in the same town and can visit, and she has 5 grandchildren. Mrs. Apolo cares for her 2- and 4-year-old grand children while one of her daughters works. In report the night nurse tells you that Mrs. Apolo is depressed and states she does not want an Ostomy, does not know how she will ever be able to care for this "pouch on my side" and is upset that she "will never be able to go in the pool". The ostomy is functioning well, and the appliance is intact. She is on IV Ampicillin 2g IV Q6H for 4 more days, most recent labs show
potassium 3.4 mmol/L and WBC 10.8 x10e9/L. She takes no other meds except OTC supplements and occasionally Tylenol Arthritis for pain.

Describe and discuss the procedure(s) or issue(s) from the scenario that may impact the recovery and health of the medical/surgical client.
Describes how to best care for this client with evidence supported by literature. (Nursing interventions.)
Discuss the reported blood work and how it may impact the health and recovery of your client with support from literature.
Identifies best health promotion teaching applicable to the client, supported by literature.
Reflect on how the nurse using best practice, evidence-supported literature promotes recovery and health of the client.
2. Through a review of the current nursing research, you will be expected to identify evidence-informed practices for addressing the client's nursing care needs. You will discuss the nursing care procedure, issue or intervention and summarize your findings, as they would apply to evidenced informed care for your client.
