CASE STUDY: ACUTE CARE CHALLENGE.

Mr. James is an 80-year-old widow who is obese and hospitalized for the past 7 days for an acute stroke. His past medical history includes hypertension, coronary artery disease, diabetes, (diet controlled), hyperlipidemia, mild congested heart failure, anemia, and osteoarthritis. He has (R) sided paralysis and is oriented to person only. Three people are needed for positioning. He is out of bed in the chair once a day. He has an NG due to swallowing problems. He is on complete enteric feeding. Labs available include: prealbumin is 8 and albumin is 2.0; A1C -9.0; Hgb- 10. He is incontinent of urine and feces. His lower legs are edematous 2+ bilat. He was place on a pressure redistribution support surface- alternating air. He moans when you reposition him and when his heels are on the surface. Family report that he lives alone at home and attendance at family activities has decrease in the past 6 months; he had Meals on Wheels daily.
