Nurse's Notes 1735 60 year old female client reporting a generalized headache with accompanying nausea and vomiting and a sore neck over the past several hours unrelieved by over-the-counter medication. Son adds that the client has become irritable and cannot stand to have the lights on. History of present illness - "I have just felt bad for several days, but it has gotten worse today." Past medical history - Type 2 Diabetic x 5 years. No known allergies. Drowsy, but oriented x 3. Reports headache pain of 9 on a 1-10 pain scale. Vital signs: T 103° F (39.4° C), P 90, RR 16, BP 118/66, pulse oximetry reading 98% on room air. Unable to touch chin to chest. Client placed supine; hips and knees flex when lifting client's head. With the hip and knees flexed, the client is unable to straighten leg without pain. Faint pink skin rash noted on the chest.
