YOUR TASK
Prepare a report for circulation and discuss with the company Directors.
The Directors don't fully understand the (UK & EU) law in relation to safety, so it will good if you could explain this in the report. Please include:
1. A summary of your company safety regulatory duties in the UK and EU with particular reference to this accident.
2. The immediate and root causes of the accident.
3. Immediate actions to prevent a recurrence.
4. Longer-term/ managementactions to betaken (control measures).
5. Issues with health and safety management system.
6. Issues with safety culture and human factor.
7. Using a table, identify all of the hazards which may arise from the operation.
8. Provide adraft action plan forpoints 2 and 3 allocating the work to the most appropriate person. Ensure you use HSE Template for Accident Investigation which you will add as your Appendix.
