Leadership requested that Nurse Managers(NMs) develop an action plan. NMs suggested the idea of purposeful hourly rounding which wasalready in practice in the medical surgical units. The NMs presented this idea to the nursing staffand the staff almost unanimously resisted the idea because they believed that it would be tootime consuming and increase the work load for nurses. The process of hourly rounding includes EVIDENCE-BASED PRACTICE PROPOSAL-SECTION A:ORGANIZATIONAL 2CULTURE AND READINESS ASSESSMENT intentionally rounding on every patient at a frequency of one hour around the clock in order toanticipate patient needs (Hall, Melusky& Toole, 2016).One of the barriers stated in an articlewas inconsistency of the performance (Hall et al, 2016). The rounding logs were completedwithout doing actual hourly rounding. Staff needed education to initiate the process. The first step was to select and train hourlyrounding champions from each shift. An extensive training was given to them to educate themabout the benefits of doing hourly rounding. Competency assessments were done for thechampions first and then done for every staff. Leadership monitored and measured the results.The Advanced Practice Nurses (APNs) are the change agent models and they are the ones thatsell the practice to improve patient outcomes. Purposeful hourly rounding generally targets “fourPs”; pain, potty or assist with toilet needs, positioning and personal equipment (Halm, 2009).While doing the hourly rounding the staff ensure that patients’ needs are met in a timelymanner. Hourly rounding is done by assisting patients in pain relief by offering pain reducingmedications, assisting with toileting, providing repositioning to avoid skin break down andpromote comfort and help to reach their persona equipment and needed items. Crucial to hourlyrounding is informing the patient that the nurse is going to come back in an hour and if anythingelse is needed to instruct the patient in the use of the call bell, always placing it in reach beforeleaving the room.Before leaving the room the nurse makes sure patients all needs are met. Bydoing this the nurse will reduce the risk of falls and increases patient satisfaction. "
