She is 73 years old lady who livesalone. She is suffering from many disorders including “COPD, hypertension, diabetes type 2,osteoporosis, cervical cancer, and anxiety”. Ten years ago, she had undergone hysterectomy forcervical cancer. She has a history of long time smoking. During her visit to the hospital, she wasgiven a treatment of Prednisone which results in unstable levels of her Blood sugar level. Beforehospitalization, all her vital signs were stable.She was a GP practice nurse and also involved inreviewing of nursing articles. As she was also suffering from Anxiety, I have to explain herabout the change in medication using communication principles. I have to discuss self- management concepts, two different types of technology related to patient-centered care duringher discharge procedure to home. I have to analyze why she is reluctant towards the advancedcare planning and advanced care directives before her discharge. I have to share the ethical andlegal principles of advanced health directive as well with her.After getting the medical history of Ms. Moly, I would recommend her to takeCorticosteroids in Inhalation dosage form. They are considered as the foremost beneficial drugsfor the treatment of patients suffering from diabetes and COPD (Herth F.J.F, 2015). Inhalationform will reduce the frequency of COPD exacerbations when compared with placebo in somestudies. They are recommended as compared to oral dosage form because they have fewer sideeffects for long-term treatment of COPD (Dhruti Pandya, 2014). A double –blind, placebo-controlled demonstrating study shows that there is no increasedrisk of Diabetes mellitus or hyperglycemia when the Inhaled corticosteroid is given to thepatients with COPD. They used Budesonide alone or a combination of Budesonide / Formoterolin COPD. Studies show that involvement of COPD and Type 2 Diabetes does not depend on the2 Student Name and Numberuse of Corticosteroids when taken in Inhalation form (O'Byrne PM, 2012). Therefore, I wouldsay Either Budesonide alone or a combination of Budesonide / Formoterol will help Ms. Moly torecover from her condition.While communicating to the patient, it is very necessary that they will listen andunderstand. It is the most accountable practice of nursing profession. Effective communicationneeds the concentration and visualization of all the verbal and non-verbal information given bythe patients. By understanding the gestures given by the patient, a nurse will get to know thecondition and he/she can increase her focus towards the care and nursing diagnostics. Personalrelationship should be god between the patient and nurse which helps the nurses to ask thequestions related to the patient’s current condition and ensure the patient by giving properinformation in such a way that will not scare the patient, will increase the trust and interest ingetting the knowledge. Courtesy and kindness should be there in nurses while havingcommunication with patients. Nurse has to gain the trust and develop the relationship withpatient so that it will results in effective communication (Lambrini Kourkouta, 2014). I will greether first and ask her about how is she feeling now? I will tell her that I know about hercontribution to human health and it is very honorable for me to help her during this time. So we are discontinuing Prednisone, and youwill get your Blood sugar level normal within few days after the discontinuation of Prednisone.We are giving the drug in Inhalation form which will result in faster recovery and have low sideeffects as compared to the oral dosage form. Since it may result in Hypertension, so your co-administration, of Atenolol will help to maintain the level. You need not worry about anything.We are here to help you and to treat you. We are not giving you Salbutamol &Tiotropium now aswe don’t want to give you more medicines for a single disorder. Alendronate & Metformin SRtreatment will be same as earlier for the treatment of Osteoporosis and Diabetes 2 respectively.We want your speedy recovery hence we are changing the medication for COPD that will resultin no impact on Diabetes level, and it has less exacerbation (Lambrini Kourkouta, 2014).Ms. Molly, I am happy to see your speedy recovery and good to see that finally, you aregoing to your home. Web advancements have been used for sickness administration innumerous clinical territories, e.g., in case of asthma and diabetes administration, where checkingmay prompt early identification of possibly basic circumstances and early mediation will helpthe patient in fast recovery.
